Prepare and submit the following copies with your application:

• CV
• Identity Document (ID)
• Certified Matric certificate
• Proof of residence (from your Ward Councillor)
• Affidavit confirming you are unemployed and not receiving any government grant

Where to Submit

• Corporate Services (Durban):
  Health Systems Trust – HR Department, 1 Maryvale Road, Westville, Durban.
• uMgungundlovu District:
  Submit your CVs into the boxes marked “CVs” at the relevant facilities listed above in the District.

Important Notes

• Personal information is processed in line with POPIA.
• Late applications will not be considered.
• Only shortlisted candidates will be contacted and will undergo reference and credential checks.
• If you have not been contacted within 15 days of the closing date, please consider your application unsuccessful.
